I push on the cardstock around the grommet until the prongs pop through the backside. A bone folder helps. To get my ribbon inside the grommet, I did not flatten the prongs; I turned the card front side & threaded ribbon through each side, easing back the grommet as needed. Last I flattened the prongs.
